Lending Library

AFFM will mail your selections to you free of charge, you, in turn, are responsible for shipping your items back. If you would like to extend the due date of the requested materials please contact us. If borrowed items are lost or damaged it is the responsibility of the borrower to replace the item.

*Please note that children’s books are not eligible for training credit.
To receive a certificate of training please provide a write up with each item returned.

Library material can be requested below by doing the following:

• Check the box to the left of the items you would like – Limit to 3 per request.
• When at least one item has been selected, a “Reserve Items” button will appear.
• Once you have selected the items you would like to reserve, click on “Reserve Items.”
• Fill out the request form and submit your request.
